Task: I took the initiative to fast-track some stages of the development process, believing that this would help us meet our deadlines and perhaps even deliver early. However, this decision meant that some critical quality assurance steps were skipped.

Action: As the project progressed, it became clear that several bugs and issues had slipped through because of the lack of thorough testing. I convened an emergency meeting with the team to address these issues, and we had to work overtime to fix them before the release.

Result: The project was delayed, and the team experienced burnout due to the increased workload. This was a major failure on my part because my decision to skip critical steps had a negative impact on both the project timeline and team morale.
